Cornwall-based Aram Resources will have to pay out more than £100,000 in fines and costs after a worker was killed at the Carnsew Quarry in 2004.
Robert Bickley, 42, died after he became caught in the fixed guard of a primary crusher machine – a machine used to break up large quarried rocks into smaller pieces – as it came into contact with the moving flywheel.
Health and Safety Executive inspector Simon Edwards said: "The company lacked the competence to identify that the guards provided for the machinery did not comply with health and safety standards."
It was fined £40,000 and £35,000 for two separate safety breaches, and ordered to pay costs of £30,000.
The company had pleaded guilty to both charges.
